When you call, please have the following ready:
- Patient full name and date of birth
- Type of records needed (consult report, progress notes, labs, imaging, etc.)
- Your contact information and preferred delivery method
Records requests are handled in accordance with HIPAA privacy requirements. Processing times may vary.
